Annual Residency Verification Has Begun

 This information must be submitted on a yearly basis and/or as needed. 

DUSD #27 is required to maintain verifiable documentation of residency for pupils who enroll in the school district as per ARS 15-802 (B)   Homeless are excluded. Please ask about the Arizona Affidavit of Shared Residence if not able to present proofs.

The Annual Residency form and current proof of residency under the parent's/guardian's name must be presented at the New Student Center during the second semester of every school year.  If student is under guardianship, you must also present the most current Order for Non-Appearance Review Hearing from the courts. 

Forms will not be accepted through mail or fax.
